If you know someone aged 15 or above with learning disabilities who would benefit from Sam's Place, you can refer them to our enterprise learning, social facilities, and activities. We are dedicated to providing a supportive and inclusive environment for individuals with learning disabilities to enhance their skills and overall well-being.
To refer someone to Sam's Place, you have multiple options:
1. Fill out the Form: You can refer by filling out the referral form. Please provide accurate and detailed information to help us understand the specific needs and requirements of the person you are referring. We will review the form and get in touch with you as soon as we can.
2. Email: Alternatively, you can refer someone by sending an email to Ann or Stephanie. Please address your email to annhsamsplace@gmail.com or Stephaniewsamsplace@gmail.com and include relevant details about the person you are referring. We will respond to your email promptly and guide you through the referral process..
3. Call : If you would like to discuss the referral over the phone, you can contact us directly. Please dial 07391 539239. We will be available to answer your questions, discuss any concerns, and guide you through the referral process.
